Constance (Connie) Westlake, born May 5, 1952, passed away on August 4, 2018 surrounded by loved ones. She was loved by all that knew her and will be truly missed.
Connie is survived by her children, Lynn (Carlos) Rodriguez, Kim (Jeff) Palmerton, Alan Chupa, Justin (Sarah) Chupa, Chrissy (Tracy) Cresong; her beloved dog, Dee; and her grandchildren, Ashley, Summer, Jacob, Tailor, Paul, Troy, Luke, Jordan, Haven (Sammi), Digi, Daisy, Mckenna, Hannan, Hannah, Braden, Victor, and Jazz; great-grandchildren, Kaisen, Noah, Addie, Amara and Hunter. In lieu of flowers, donations may be sent to St. Jude Children's (800) 822-6344.
Family will be having a gathering on Saturday August 18, 2018 from 1pm-7pm. Dinner at 4pm. at 12797 Oakwood Ave. NW Uniontown, OH 44685. Please stop by and share your memories.
